Long Term Care/ Subacute experience preferred.
Attend and participate in interdisciplinary care plan meetings.
Complete MDS while developing and maintaining individual care plans.
Interview and access residents’ needs during the admission interview process.
Work with the team to coordinate discharges.
Work with residents to maintain psychosocial well-being.
