Social Worker / Counselor- Behavioral Health

Duties Responsibilities: Assistphysician and health care team members in understanding the significant social and emotional factors related to health problems. Participatein the development of the plan of treatment. Prepareclinical and progress notes. Workwith the family; utilizes appropriate community resources; leads patient groups; and participates in discharge planning. This position will be assigned to the Behavioral Health Unit and work with both the geriatric and adult populations. Social Worker Qualifications include: Bachelors degree in social work required, Masters preferred. South Carolina license in Social Work required Knowledge regarding community resources required. One year of hospital/health care experience preferred. Excellent communication skills and an ability to work in a collaborative team approach are required. Counselor Qualifications include: Masters Degree in Counseling or related field. LPC or LMFT required.
