This Social Worker Case Manager (MSW) position will be assigned to the Neonatal Intensive Care Unit (NICU) and work a (10) ten hour shift schedule Sunday-Wednesday, 40 hours weekly full time).

Job Summary and Qualifications

The Medical Social Worker/ Case Manager evaluates psychosocial needs and provides community service coordination for identified patient populations, providing support and crisis intervention where indicated. The SW Case Manager acts as a liaison between the patient and community resources/post-acute care providers. The SW Case Manager creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The SW Case Manager facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.

QUALIFICATIONS:

  • State licensure required
  • Minimum of two (2) years full-time social work experience preferred.
  • Medical social work experience preferred.
  • Certification in social work or case management preferred
  • Knowledgeable about internal, community and post-acute care resources
  • Basic social worker skills, including but not limited to, empathy, sensitivity to cultural and ethnic diversity, advocacy, communication skills and crisis intervention

EDUCATION:

  • Graduate of a school accredited by the Council on Social Work Education.
  • MSW (Master’s Degree in Social Work or equivalent) or BSW with commensurate experience required.
  •  EXPERIENCE: Minimum of two (2) years full-time social work experience (min 3 years) preferred.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Excellent oral and written communication skills.
  • Excellent psychosocial assessment and documentation skills.
  • Basic computer skills required.
  • CPR required within thirty (30) days of hire.
  • Complies with hospital employee health, infection control and safety management programs.

LICENSURE:

  • Current State of Nevada Board of Examiners for Social Work license;
  • Independently maintains license; Acquires required continuing education units to practice and maintain license.
