    Summary

    The Social Worker provides direct psychosocial support, assessment, care planning, and discharge/transition coordination for patients and families. Working collaboratively with the interdisciplinary team, the role addresses social, emotional, and practical barriers to care and connects patients with appropriate community resources.

    Essential Functions

    • Screen patients for social work needs and complete psychosocial assessments as needed.
    • Provide counseling, care coordination, and transition planning.
    • Collaborate with the care team to support safe, timely discharge and optimal outcomes.
    • Maintain accurate, timely documentation.
    • Connect patients and families to community and referral resources.
    • Assist with family meetings, guardianship, abuse/neglect concerns, psychiatric referrals, end-of-life issues, and charity care.
    • Participate in rounds, quality improvement, supervision, and on-call/coverage duties as needed.

    Qualifications

    • MSW required
    • Maryland LMSW required
    • Strong communication, collaboration, assessment, and organization skills
    • Healthcare or field placement experience preferred

    Salary Range: Minimum 32.40/hour - Maximum 53.48/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
