Pay Range:
$28.14 - $42.21A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.
Education
Master’s Degree in social work is required.
Bachelor's degree in social work plus five years of social work or social service clinical experience may be considered in lieu of a master's degree.
Experience
Prior experience in social work or past social service clinical experience directly transferrable to a healthcare environment is preferred.
2 years of home care, hospice, discharge planning or related experience is preferred.
Certifications Licenses and Registrations
Licensed as a Certified Social Worker (CSW) in the state of Wisconsin or Licensed Social Worker (LSW) in the state of Illinois is required.
Wisconsin: Licensed as a Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) is preferred. Illinois: Licensed as a Licensed Clinical Social Worker (LCSW) is preferred.
Eligible temporary Social Worker license while completing the examination requirements for full licensure is required. Social Worker licensure must be obtained within the timeframe permitted under the temporary license and as required by applicable state regulations.
Basic Life Support (BLS) is required.
In roles which travel is required, a valid driver's license, automobile insurance and own transportation is required.
