    Job Summary
    We are seeking a qualified Hospital-Based Social Worker to provide support in a behavioral health services (BHS) environment. This role plays a critical part in patient care coordination, discharge planning, and psychosocial assessments within a hospital setting. Candidates must be licensed (LCSW) in Georgia and have active BLS certification.

    Key Responsibilities
    • Provide clinical social work services in a hospital-based behavioral health unit.
    • Conduct psychosocial assessments and coordinate with interdisciplinary medical teams.
    • Develop care plans and provide emotional support to patients and families.
    • Support patient transitions and discharge planning with internal and external stakeholders.
    • Maintain timely documentation aligned with hospital and regulatory standards.

    Required Experience
    • Minimum 2 years of experience as a hospital-based or medical social worker.
    • Familiarity with acute care and behavioral health environments.

    Required Skills
    • Active LCSW license in the state of Georgia.
    • Current BLS certification.
    • Strong crisis management and intervention skills.
    • Effective interpersonal and communication abilities.

    Nice-to-Have Skills
    • Experience working within hospital electronic medical record (EMR) systems.
    • Familiarity with discharge planning for behavioral health patients.
