Job Summary
We are seeking a qualified Hospital-Based Social Worker to provide support in a behavioral health services (BHS) environment. This role plays a critical part in patient care coordination, discharge planning, and psychosocial assessments within a hospital setting. Candidates must be licensed (LCSW) in Georgia and have active BLS certification.
Key Responsibilities
Provide clinical social work services in a hospital-based behavioral health unit.
Conduct psychosocial assessments and coordinate with interdisciplinary medical teams.
Develop care plans and provide emotional support to patients and families.
Support patient transitions and discharge planning with internal and external stakeholders.
Maintain timely documentation aligned with hospital and regulatory standards.
Required Experience
Required Skills
Active LCSW license in the state of Georgia.
Current BLS certification.
Strong crisis management and intervention skills.
Effective interpersonal and communication abilities.
Nice-to-Have Skills
