Andover Forest Post Acute Care Center is seeking a compassionate and organized Social Work Assistant to support residents and their families in our skilled nursing facility.
Responsibilities
Assist with resident and family support services
Help coordinate social services, discharge planning, and resident needs
Maintain accurate documentation and records
Communicate with residents, families, and the interdisciplinary care team
Assist with Medicaid-related processes
Qualifications
Strong communication, organizational, and interpersonal skills
Basic knowledge of the Medicaid process required
Skilled nursing facility or long-term care experience preferred
Social services experience preferred; a degree in social work or a related field is not required
Pay: $45,000-$50,000 per year
