Summary:
The Social Services Worker-MSW is responsible for working with the interdisciplinary team to affect overall care coordination. The Master's-Level Social Worker provides comprehensive behavioral health and psychosocial services to patients receiving care in outpatient hospital clinics. The Social Worker collaborates with physicians, advanced practice providers, nurses, therapists, case managers, and other members of the interdisciplinary care team to address patients' emotional, behavioral, social, and practical needs. This position provides assessment, brief intervention, counseling, care coordination, resource linkage, crisis intervention, and discharge/follow-up support as appropriate to the outpatient setting. The Social Worker serves as a patient advocate and helps reduce barriers to treatment while promoting overall health, wellness, safety, and continuity of care.
The Social Services Worker-MSW must demonstrate the knowledge and skills necessary to provide age-appropriate patient care and is accountable to the policies and procedures of the organization. The Social Services Worker-MSW is expected to adhere to the hospital and social work codes of ethics.

Neonate (birth - 28 days)
Infant (29 days - less than 1 year)
Pediatric/Child (1 year - 12 years)
Adolescent (12 years - 18 years)
Adult (18 years - 65 years)
Geriatric (over 65 years)
Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

EDUCATION (Minimum Requirements for Position):
Master's Degree required.
EDUCATION FIELD OF STUDY (If Applicable):
Mental Health related field required.
CERTIFICATES, LICENSES, REGISTRATIONS:
Non-Violent Crisis Intervention program obtained within six full months from date of hire (Travelers-required upon hire)

EXPERIENCE:
Previous clinical experience preferred.
Skills/Knowledge Requirements:
SKILLS:
Language Skills - Ability to read, write, speak, and understand the English language required.
Other Certifications/Requirements:
Must be 21 years of age to apply required.
