About the Agency

About the Position

Third-level professional position accountable for discharge planning and coordination of services for individuals admitted to MSH male receiving services. This includes and is not limited to: biopsychosocial assessments, treatment plans and participation in treatment team meetings, documentation of patient progress and other required documentation, referrals and coordination of aftercare treatment, discharge education groups, communication with families/correspondents, community providers and facilities/agencies, law enforcements, and attorneys. Positions at this level may perform the duties described at the lower levels. At this level, employees must demonstrate expertise in a particular area of social service (e.g., psychiatric, medical, etc.) OR in specific areas such as case management or knowledge of community resources. At this level, employees may be required to assist with work coordination, case review, consultation, technical assistance, policy development, and training to the lower levels.

Successful candidates must have a good attitude, be ready and willing to learn and grow, have strong attention to detail, strong communication skills, and excellent organizational skills. Employees must possess a thorough knowledge of social service policies, practices, and techniques, having mastered the technical job content, perform work of greater complexity, exercise greater independence in making decisions, excellent verbal and written communication skills, the ability to multi-task, and have an established reputation and credibility both within the agency and community.

Preferred Qualifications

Typically requires a Bachelor's Degree and 3-5 years of experience. Appointing authority may require social worker's license.
