Pay Rate:

• Travel: $2,340–$2,440/week

• Local: $2,000–$2,100/week

Job Description:
Provide medical social work services in an acute care hospital, including psychosocial assessments, discharge planning, care coordination, patient support, and connecting patients with post-acute and community resources.

Responsibilities:

• Perform psychosocial assessments.

• Develop and coordinate discharge plans.

• Assist with post-acute care planning.

• Connect patients with community resources.

• Identify and address barriers to safe discharge.

• Coordinate with physicians, nurses, case managers, and other healthcare staff.

• Support patients and families.

• Maintain accurate clinical documentation.

• Participate in interdisciplinary care planning.

Required Qualifications:

• Master of Social Work (MSW) required.

• Minimum 2 years of acute medical Social Work experience.

• Social Work and Discharge Planning experience required.

• Acute Care hospital experience required.

• Strong patient assessment and care coordination skills.

• First-time travelers accepted with additional screening.
