Essential Functions/Duties/Responsibilities:

  • Lead and manage the social service department to ensure the desired level of resident service, care, and satisfaction. Ensure equitable administration of policies, and compliance with regulatory and legal requirements.
  • Establish a trusting relationship with the residents and their families to orient and fully acquaint them with resources (i.e. Medicare and other insurance benefits) available throughout the continuum of services, the billing processes and resident financial responsibilities, and the specific schedule of services (i.e. mealtimes, bathing, etc.) in order to create a smooth transition from admission to discharge, including changes in level of care for the resident.
  • Assure that an initial social service assessment of each resident entering the Health Centers is completed, to include a history and current level of social and emotional needs. Assure that the Minimum Data Set (MDS) required interviews are conducted, within the required amount of time, in order to assure proper levels of support are provided and to remain in compliance with federal, state, and community requirements.
  • Participate in the Health Services Interdisciplinary Team (IDT) by scheduling and conducting weekly family care conferences and provide input to the weekly Medicare meeting, monthly triple check, and Prospective Payment System (PPS)/discharge planning meeting; and communicate with residents' families regarding the discharge process and post-discharge care requirements to achieve a smooth transition from the current level of care or from Montereau.
  • Identify and maintain effective relationships with agencies and service resources available within the local community (i.e. medical, financial, social, supplies, etc.) to assist residents and/or family members in obtaining necessary assistance upon their discharge from Montereau.
  • Participate in various interdisciplinary team meets such as Transitions of Care, Standards of Care, and QAPI by identifying physical and psychosocial needs of the resident and assist the family in planning to address these needs.
  • Assure that residents and families are aware of their rights and grievance procedures and proactively work to address issues before they become a source of dissatisfaction; and provide an advocacy outlet for Montereau residents. Ensure the grievance log is kept up to date.
  • Participate in Leader on Duty rotation.
  • Assist as a backup with Health Center marketing efforts by providing tours to prospective residents and families to support Montereau in meeting its census targets.
  • Ensure proper and timely documentation is entered into the resident's medical record to include progress notes, care plan updates, advance directive updates, family communications, required MDS sections, and bed hold policy's requirements.
  • Organize the entire discharge process to include, but may not be limited to, issuing the resident NOMNC (Notice of Medical Non-Coverage) and ABN (Advanced Beneficiary Notice), faxing referrals to other communities for LTC placement, ordering durable medical equipment, coordinating home health and hospice services, communicating anticipated discharge dates to managed care insurance companies and hospital ACO's.
  • Schedule Podiatry visits and assist with diabetic shoe orders.
  • Communicate with Adult Protective Services, when needed.
  • Participate in Caring Partner Rounds.
