We are seeking a compassionate, organized, and resident-focused Social Services Director to join our skilled nursing and long-term care team. This position plays an essential role in supporting the emotional, social, and psychosocial well-being of our residents while helping residents and their families navigate care decisions, transitions, and available resources.
Assess residents’ psychosocial needs and develop appropriate care plans
Participate in interdisciplinary care plan meetings
Provide support and guidance to residents and their families
Assist with admissions, discharges, transfers, and discharge planning
Coordinate community services, referrals, transportation, and other resources
Advocate for residents’ rights, preferences, dignity, and quality of life
Address resident and family concerns promptly and professionally
Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements
Assist residents with advance directives and related care-planning needs
Support residents experiencing grief, adjustment difficulties, behavioral concerns, or changes in condition
Collaborate with nursing, therapy, activities, dietary, and other departments
Participate in quality assurance activities and state or federal surveys
Maintain confidentiality and comply with HIPAA and all applicable regulations
Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred
Previous social services experience in skilled nursing, long-term care, healthcare, or a related setting preferred
Knowledge of residents’ rights, discharge planning, and long-term care regulations is a plus
Strong communication, documentation, organization, and problem-solving skills
Ability to work compassionately and professionally with residents, families, staff, and community providers
Must be dependable, detail-oriented, and able to manage multiple priorities
