Social Services Director

We are seeking a compassionate, organized, and resident-focused Social Services Director to join our skilled nursing and long-term care team. This position plays an essential role in supporting the emotional, social, and psychosocial well-being of our residents while helping residents and their families navigate care decisions, transitions, and available resources.


Key Responsibilities

  • Assess residents’ psychosocial needs and develop appropriate care plans

  • Participate in interdisciplinary care plan meetings

  • Provide support and guidance to residents and their families

  • Assist with admissions, discharges, transfers, and discharge planning

  • Coordinate community services, referrals, transportation, and other resources

  • Advocate for residents’ rights, preferences, dignity, and quality of life

  • Address resident and family concerns promptly and professionally

  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements

  • Assist residents with advance directives and related care-planning needs

  • Support residents experiencing grief, adjustment difficulties, behavioral concerns, or changes in condition

  • Collaborate with nursing, therapy, activities, dietary, and other departments

  • Participate in quality assurance activities and state or federal surveys

  • Maintain confidentiality and comply with HIPAA and all applicable regulations

Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred

  • Previous social services experience in skilled nursing, long-term care, healthcare, or a related setting preferred

  • Knowledge of residents’ rights, discharge planning, and long-term care regulations is a plus

  • Strong communication, documentation, organization, and problem-solving skills

  • Ability to work compassionately and professionally with residents, families, staff, and community providers

  • Must be dependable, detail-oriented, and able to manage multiple priorities
