Position Summary: Social Services Director
The Social Services Director is responsible for planning, organizing, developing, and directing the facility's social services program to ensure the emotional, psychosocial, and social needs of residents are met. This position works closely with residents, families, nursing staff, and the interdisciplinary care team to support resident well-being, coordinate discharge planning, address concerns, and promote the highest possible quality of life.
Key Responsibilities
- Lead and coordinate the facility's Social Services Department.
- Assess residents' psychosocial, emotional, and social needs and develop individualized care plans.
- Advocate for residents' rights, dignity, independence, and overall well-being.
- Provide emotional support, counseling referrals, and resources to residents and families.
- Coordinate discharge planning, including community resources, referrals, and transition services.
- Participate in resident assessments, care plan meetings, and interdisciplinary team discussions.
- Assist residents and families with adjustment to long-term care and changes in health or living arrangements.
- Address resident and family concerns while promoting positive communication and resolution.
- Maintain accurate documentation and ensure compliance with federal, state, and facility regulations.
- Participate in quality assurance, performance improvement (QAPI), and survey readiness initiatives.
- Build relationships with community agencies and outside service providers to support resident needs.
- Promote a compassionate, resident-centered environment focused on dignity, respect, and quality of life.
Qualifications
- Degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
- Previous experience in social services, preferably in a skilled nursing facility or long-term care setting.
- Knowledge of resident rights, discharge planning, psychosocial assessments, and applicable healthcare regulations.
- Strong communication, organizational, problem-solving, and interpersonal skills.
- Ability to work collaboratively with residents, families, and interdisciplinary healthcare teams.
- Compassionate, dependable, and committed to improving residents' quality of life.
- Must meet all applicable state and federal qualification requirements for the Social Services Director position.
