Description

We are partnering with an assisted living community in their search for a temporary Social Services Coordinator.  In this role, located in Boca Raton, FL, you will serve as a vital liaison and advocate for residents across Independent Living, Assisted Living), and Memory Care. Working alongside an interdisciplinary team, you will assess resident needs, build strong personal relationships, coordinate health and wellness resources, and ensure seamless transitions throughout the continuum of care.

Job Responsibilities:
  • Serve as a key liaison and advocate among residents, family members, caregivers, and internal/external care providers to promote resident-centered outcomes.
  • Assess residents’ physical, emotional, and social needs continuously through direct observations, consultations, and team referrals.
  • Guide and support residents and families during health and wellness transitions between levels of care (Independent Living, Assisted Living, Memory Care, Respite, and Rehab).
  • Conduct empathetic follow-ups with residents post-hospital discharge to provide necessary resources, direct concerns, and drive high resident satisfaction.
  • Assist residents and families with understanding health plan benefits, insurance queries, and Advanced Directives.
  • Monitor hospitalizations, ER visits, falls, and significant incidents in collaboration with the emergency medical response team.
  • Maintain and update daily non-medical case reports and shared interdisciplinary tracking lists regarding resident statuses and transitions.
  • Attend and contribute to key community meetings, including Interdisciplinary Team Level of Care Transitions, Assisted Living Stand-Ups, Care Coordination, and Health & Wellness Committees.
  • Facilitate New Resident Orientations, welcome new residents, explain available support services, and encourage engagement in social and community programs.  
  • Develop and maintain a comprehensive directory of internal and external health resources, vetting them to align with resident and community needs.
  • Partner with external educators, instructors, and healthcare professionals to plan and contract health education programs, support groups, and wellness initiatives.
  • Support a whole-person approach to health and wellness across eight key dimensions: Emotional, Environmental, Health Services, Intellectual, Physical, Social, Spiritual, and Vocational.

Requirements:
  • Education: Bachelor’s degree or higher in Social Work (BSW/MSW) or a closely related human services field.
  • Experience: Experience in senior living, post-acute care, case management, or social services coordination within a multi-level care environment preferred.
  • Core Competencies: High emotional intelligence, exceptional communication skills, strong organizational abilities, and deep empathy for the senior population.
  • Physical Requirements: Ability to sit, stand, walk, and bend intermittently throughout the day. Ability to safely lift/move up to 25 lbs. and occasionally assist residents from a seated to standing position.
