Key Responsibilities:

  1. Resident Advocacy: Listen to residents’ concerns, preferences, and needs; communicate these to the care team; advocate for accommodations or care preferences
  2. Emotional Support: Provide comfort and companionship to residents, helping them cope with loneliness, anxiety, or depression
  3. Care Planning & Coordination: Assist in developing and updating residents’ care plans; coordinate additional services such as counseling or therapy
  4. Family Communication: Serve as a liaison between residents and families, providing updates on well-being and answering questions
  5. Admission & Orientation: Help with the admission process, interview potential residents and families, complete assessments, and orient new residents to the facility
  6. Documentation: Maintain accurate, confidential records; complete assessments, discharge plans, and care plan conferences
  7. Problem Resolution: Facilitate resolution of residents’ and families’ concerns through counseling, problem-solving, and coordination with other departments
  8. Community Resources: Connect residents with community services and benefits

Qualifications:

  1. Education: Often a bachelor’s degree in social work, psychology, sociology, family services, or a related field; some facilities may accept equivalent training
  2. Experience: Prior experience with geriatric populations and institutional settings preferred
  3. Skills: Strong communication (oral and written), empathy, organizational skills, ability to document in computer systems, and knowledge of medical terminology
  4. Other: Valid driver’s license, ability to pass background checks, and willingness to work evenings/weekends
