    Description

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    • Core duties and responsibilities include the following. Other duties may be assigned.

    • Monitors and evaluates the provision of client services in the social service program area.

    • Leads the patient advocate and transportation teams; evaluates staff performance and recommends appropriate action. Trains and assists patient advocate and transportation staff.

    • Fosters effective community-clinic relations and organizes community projects.

    • Acts as the social services liaison for the medical and behavioral health care home teams.

    • Is responsible for acting as a social resource expert at NACC assisting providers and other staff with patient's social and case management needs.

    • Assigns social service related tasks to patient advocate staff and ensures timely completion.

    • Delegates and coordinates patient advocate team participation in community-based work, groups and stakeholder meetings.

    • Facilitates regular meetings with patient advocate team and compiles monthly submissions from the team.

    • Monitors department budget and keeps management appraised of budget deviations.

    • Coordinates interdepartmental communication on behalf of the patient advocate and transportation teams.

    • Oversees data collection and management to ensure adequate data reporting to grantors (MNSure, other patient encounters and community outreach engagements).

    • Oversees patient advocate and transportation work performance and attendance as outlined in the NACC employee handbook.

    • Participates in community meetings and outreach events. Performs in-reach and out-reach to connect patients and community with resources.

    • Maintains current community resource information in an organized and accessible format. Meets with patients in-clinic to assess and assist with the patient's social needs.

    • Assists patients in completing medical insurance applications.

    • Participates in regular social services grant meetings with Community Health Manager and funders.

    ACCOUNTABILITY:

    • Is accountable for managing grant deliverables and reports as well as managing grant expenses (purchase orders). Maintains an up-to-date grant budget report for MNsure and the federal outreach and enrollment grant.

    • Is accountable for enrolling uninsured patients for medical insurance and maintaining up-to-date community resources.

    SUPERVISORY RESPONSIBILITIES:

    Supervises 3-5 staff members of the patient advocate and transportation teams
