Social Care Navigator FT

The Social Care Navigator Tier I will systematically address the Health Related Social Needs (HRSN) of Medicaid members with and without Intellectual and/or Developmental Disabilities by providing screening, eligibility, and navigation services that will identify any HRSNs of the Medicaid member. The Social Care Manager will promote confidentiality, privacy, human dignity, uniqueness, and physical and emotional well-being while supporting people to lead a self-directed life. The Social Care Manager will support people to develop and maintain relationships, advocate for justice, fairness and equity, and full community participation.

Minimum Education and Experience

• Associate's degree in a human services field with one year experience in a human services position.
• The preferred candidate will have experience working with people with Intellectual and/or Developmental Disabilities, care management experience, and a working knowledge of community supports.

Licensure/Certification

• Valid NYS driver's license required.
• Must maintain current required certifications as designated by the agency including but not limited to: First Aid/CPR

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, internal equity, and location.
