SNF Case Manager

About the Role

We are currently seeking a highly motivated RN/LVN Case Manager to support our Health Services - Integrated Case Management (ICM) team. This role reports to the Manager, Case Management and will focus on coordinating care transitions and case management services for members in Skilled Nursing Facilities (SNF), Sub-Acute (SA), Acute Rehabilitation Units (ARU), and Long-Term Acute Care Hospitals (LTAC).

This is an excellent opportunity for a licensed nurse who is interested in transitioning into case management. We are willing to train candidates with strong clinical experience and a passion for improving patient outcomes.
