Position Summary The SNAP Lead Case Manager works closely with the Program Manager and clients who have been referred by DHS to participate in the SNAP Employment & Training Program. The goal of the program is to assist clients with a path to employment.
The SNAP Case Manager is responsible for providing implementation assessments, career pathways, and overall assessment of individual SNAP customers. The case manager will work with clients throughout the initial intake visit until the client is exited from the program. The case manager collaborates with customers on job readiness skills, career assessment, and planning, as well as assisting with opportunities to complete GED or further their education. Assess and determines program eligibility for new referrals with Program Manager. Assist program referrals to complete necessary paperwork during orientation process and is responsible for interviewing, assessment, and placement of participants.
Essential Duties and Responsibilities
Hours of Work Monday - Friday, 8:30 am - 5:30 pm
Education Requirements Bachelors Degree in related fields of social services, human resources, business, sociology, or business equivalent with minimum one year of experience in working with workforce development in a for-profit setting.
