About the Opportunity

A reputable behavioral health organization in Phoenix, AZ is seeking a Serious Mental Illness (SMI) Case Manager for a temporary assignment lasting 3+ months. This role is ideal for professionals passionate about supporting vulnerable populations while gaining valuable experience in a structured clinical environment.

Although no driving is required, candidates must possess a valid driver's license per program and compliance requirements.

Key Responsibilities

• Provide comprehensive case management services to adults diagnosed with Serious Mental Illness (SMI)
• Coordinate care plans, services, and community resources to support client stability and recovery
• Conduct intakes, assessments, service planning, and documentation in accordance with program standards
• Collaborate with clinical staff, providers, and community partners
• Maintain accurate and timely case notes while ensuring compliance with agency and regulatory guidelines

Qualifications

• Previous experience in case management, behavioral health, social services, or a related field
• Familiarity with SMI populations strongly preferred
• Strong documentation, communication, and organizational skills
• Valid driver's license required (no driving duties involved)
• Ability to commit to a temporary assignment of 3+ months
