Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans.
- Administer medications, treatments, and procedures as prescribed by physicians.
- Monitor and document patient progress, reporting any changes to the healthcare team.
- Educate patients and their families on disease management and health maintenance.
- Collaborate with interdisciplinary teams to coordinate care and improve patient outcomes.
- Maintain compliance with all regulatory and safety standards.
Requirements
- Valid Registered Nurse (RN) license in the state of Michigan.
- Minimum of 2 years of nursing experience, preferably in home care or related field.

- Reliable transportation to travel to patient homes.
