Personalized, Leading-Edge Care
Helen Porter Rehabilitation & Nursing is the only skilled nursing center in Addison County, providing exceptional care close to home for the communities we serve. Residents and families consistently express high satisfaction with the compassionate, personalized care delivered by our dedicated team.
As part of the University of Vermont Health Network, Helen Porter offers a unique opportunity to lead a highly respected organization committed to innovation, excellence, and resident-centered care. We are seeking an experienced and visionary Site Administrator to guide our next chapter of growth and service.
At Helen Porter, we believe each resident is an important and valued member of our vibrant community. We are committed to caring for the physical, emotional, and spiritual well-being of every individual we serve. Residents are supported in one of three specialized neighborhoods designed to meet their unique needs:
A dedicated neighborhood for residents living with dementia and memory loss. Our activity-based care model provides a warm, supportive, and secure environment that promotes dignity, engagement, and quality of life.
Focused on rehabilitation and recovery following surgery, illness, injury, or hospitalization. Our interdisciplinary team works collaboratively to help residents regain independence and return to their homes or community settings whenever possible.
Providing 24-hour skilled nursing care and assistance with activities of daily living, while fostering meaningful community connections and resident engagement through activities, events, and personalized programming.
Helen Porter's Memory Care and Long-Term Care neighborhoods also feature dedicated end-of-life suites designed to provide comfort, privacy, and support for residents and their loved ones. Through collaboration with UVM Health Network's palliative care teams and our Palliative Support Services program, we provide compassionate care that honors each individual's journey.
The Skilled Nursing Center Site Administrator (SA) is responsible for creating and maintaining an environment that delivers high-quality clinical care in a homelike setting while ensuring compliance with all organizational policies, procedures, and federal and state regulations governing long-term care facilities.
This leader serves as a champion for long-term and post-acute care services, combining healthcare expertise, operational excellence, and compassion to support residents, families, and staff. The Site Administrator plays a critical role in advancing UVM Health Network Continuing Care's strategic priorities related to access, affordability, quality, and community health.
The SA will also contribute to the ongoing improvement and evolution of post-acute and long-term care services, helping identify opportunities to better meet the changing needs of the communities we serve.
