Shared Market Clinical - Licensed Master Social Worker

Duties/Responsibilities:

Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each memberPlan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriersNavigate managed care plans for community services and programsCase management for social and behavioral care to allow members to self-manage health and social service supportAccurate and timely documentation of patient encounters and sessions in all clinical management systemsProactively identify methods to improve ArchWell Health's approach based on feedback and regularly conducted surveysSupport advance care transitions with members and their familiesAssist members with access to state-based prescription programs and other benefits

Required Skills/ Abilities:

Experience supporting patients with need for social servicesExperience with screening, assessment, and planning for common social services needsWorking knowledge of differential diagnosis of common mental health conditionsStrong interpersonal communication skills with exceptional active listening abilitiesHighly empathetic, non-judgmental, and open-mindedExperience in a collaborative team environment

Education and Experience:

Fully licensed LMSW in the desired State of practiceMaster's degree in Social Work1+ years' experience in clinical social workA problem-solving orientation and a flexible and positive attitudeMission driven and motivated to join an organization that will transform the way we deliver accessible, clinically excellent care to seniorsProficient PC skillsFluency in Spanish or other languages spoken by people in the communities we serve (where necessary)
