    Required Skills/ Abilities:

    Experience supporting patients with need for social servicesExperience with screening, assessment, and planning for common social services needsWorking knowledge of differential diagnosis of common mental health conditionsStrong interpersonal communication skills with exceptional active listening abilitiesHighly empathetic, non-judgmental, and open-mindedExperience in a collaborative team environment

    Education and Experience:
