American Senior Communities is now hiring a Senior Transitional Care Coordinator

Senior Transitional Care Coordinator (STCC) provides customers with facility-related information via telephonic conversations and assists in coordinating care and services for new residents. This role provides coaching and mentorship to the Care Coordination Center team members, while receiving inquiries and referrals, validating healthcare coverage through all insurance plans, and engaging the appropriate team members to initiate clinical assessments and managed insurance authorization.

Requirements

  • Bachelor’s degree marketing or related field preferred; or three-years equivalent experience in lieu of degree.
  • One to three years nursing facility, community relations, sales and/or social service experience required.
  • Previous health care admissions/marketing/sales experience preferred.
