Senior LTSS Service Care Manager (RN)

Summary:

• Location: McAllen location, travel required for face-to-face visits
• Duration: 6 months
• Shift: Monday to Friday, 8 AM to 5 PM, possibility for overtime and Saturdays, flexibility required

Responsibilities:

• Identify any needs new members may have and continue care for anything placed by prior MCO
• Prepare for visits by gathering information on members using the Electronic Medical Record system, reviewing diagnosis, medications, and recent authorizations
• Complete visits that are: Initial, Change in condition, MDCP, and Level 3 reassessments
• Travel to member home for in-person visit lasting 2-4 hours; travel can be up to 2 hours one way
• Submit assessment within 24 hours; initiate any requests for services within 3 business days; complete all other documentation within 5 business days
• After 15 business days, hand off member to appropriate level to be assigned to permanent care manager/service coordinator

Requirements:

• Education/Certification: Required: RN ASN, BSN, or MSN
• Licensure: Required: RN
• More than 3 years as a nurse and 2 years of case management experience
