The primary functions of an LPN in assisting with assessment, planning, implementing and evaluating the care of all patients, regardless of age, during his/her shift. Responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. Effectively provides Person Centered care and services.Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with Facility Policy.Taking the patient's vital signs such as respiration, temperature, blood pressure, and heart rate and reporting changes in the patient's /residents conditionsPractices ethically and professionally within their scope; advocates for patientMaintains clinical competence specific to their care area including organization required education within stated timeframeCoordinates care with interdisciplinary teamDocuments appropriately and thoroughly in EMR/Point Click Care. Prefer experience in Point Click Care.Keeps other nursing personnel and responsible RN informed regarding changes in condition of patients/ residents and / or other pertinent observations
