School Social Worker (High School Transition Program)

Birch Agency is seeking a dedicated
in-person School Social Worker to support students within a school district in
Kankakee, Illinois. This position focuses on working with students at the
high school transition level, helping them develop the social, emotional, behavioral, and life skills needed for success in school, post-secondary education, employment, and independent living.
Position Overview
Birch Agency is seeking a compassionate and qualified School Social Worker to provide direct and consultative services in an educational setting. This role offers the opportunity to collaborate with educators, families, and multidisciplinary teams to support students as they prepare for life after high school and achieve their transition goals.
Position Details
Full-time, in-person position
Assigned to a high school transition program
Work with students preparing for post-secondary education, employment, and independent living
Manage a caseload of students requiring social-emotional and behavioral support
Collaborate closely with teachers, administrators, special education staff, and community agencies
Strong administrative and multidisciplinary team support
Key Responsibilities
Provide individual and group counseling services to students.
Support students' social-emotional, behavioral, and transition-related needs.
Participate in IEP meetings and contribute to transition planning and goal development.
Conduct assessments and collect data related to student social-emotional functioning.
Collaborate with families, school personnel, and community resources to support student success.
Assist with crisis intervention and behavioral support planning as needed.
Maintain accurate documentation and ensure compliance with state and federal regulations.
Help connect students and families with community-based services and post-secondary resources.
Qualifications
Valid Illinois Professional Educator License (PEL) with School Social Work endorsement.
Experience working in school-based settings preferred.
Knowledge of special education laws, procedures, and transition planning.
Strong communication, collaboration, and case management skills.
Ability to work effectively as part of a multidisciplinary team.
