Position OverviewWheeler Staffing Partners is seeking a detail-oriented and organized
Home Health Scheduler to support daily scheduling operations for clinical staff and patient visits. This role is critical to ensuring continuity of care through efficient coordination, real-time schedule management, and effective communication with staff, patients, and families.
The ideal candidate thrives in a fast-paced environment, excels at multitasking, and has a strong understanding of healthcare scheduling workflows and compliance standards.
Key Responsibilities
Coordinate and manage daily schedules for clinical staff and patient visits
Schedule start-of-care visits within required timeframes
Assign visits based on care plans, staff availability, and geographic efficiency
Verify plan of care approvals prior to scheduling services
Schedule supervisory visits in compliance with regulatory guidelines
Ensure therapy reassessments are scheduled according to required timelines
Finalize weekly schedules by end of business day each Friday
Update schedules for missed, rescheduled, or reassigned visits and communicate changes
Adjust schedules throughout the care episode based on patient and service updates
Monitor hospitalized patients and coordinate scheduling upon discharge
Review documentation and coordination notes for accuracy
Optimize staff utilization through efficient scheduling and routing
Collaborate with clinical leadership to ensure productivity expectations are met
Communicate scheduling issues or operational concerns to management
Maintain confidentiality and compliance when handling patient information
Reporting & Analytics
Generate and review daily and weekly scheduling and operational reports
Monitor visit status, staff responses, and scheduling gaps
Review utilization and service reports to ensure efficiency and compliance
Administrative Responsibilities
Process electronic workflows and administrative tasks within required timelines
Answer phones and communicate professionally via phone, email, and written correspondence
Participate in meetings, training sessions, and development activities
Assist with on-call rotation as needed
Perform general office duties and special projects as assigned
Qualifications
Minimum of 1 year of scheduling or related experience required
Home health or healthcare scheduling experience preferred
Experience with electronic medical record systems preferred
Strong organizational and multitasking abilities
Excellent communication and customer service skills
Ability to manage high-volume scheduling in a fast-paced environment
High attention to detail and accuracy
Education
High School Diploma or GED required
Technical Skills
Proficiency in Microsoft Office (Excel, Word, Outlook)
Familiarity with scheduling systems and healthcare workflows
Additional Requirements
Successful completion of background check
Drug screening may be required
