General Description
Under the supervision of the Ryan White Case Management Coordinator, the Medical Case Manager coordinates the medical care of clients living with HIV/AIDS, in conjunction with the Medical Management Clinic at LCHD/CHC and works with clients to develop a supportive relationship while facilitating access and use of available services. Work involves direct client contact and is performed in accordance with state, local agency and grant policies and procedures.
Scheduled Hours: 37.5 hours per week
Essential Functions
Knowledge Skills Abilities and Education Required
Master's degree, bachelor's degree, Registered Nurse, or associate's degree.
With a Master's/bachelor's degree, a degree in a human services field (public health, health education, social work, psychology, sociology, early child development, etc.) or a degree in non-human services field and two years case management experience.
With an associate's degree in a human services field, four years of work experience working with the public, preferably in a related field (public health, health education, social work, psychology, sociology, early child development, case management, etc.).
Bilingual Spanish written and verbal skills preferred, not required.
