    What You'll Do:

    The Center of Public Health Excellence (CPHE) is seeking one (1) dedicated Human Services Program Consultant to support the Rural Health Transformation Program (RHTP), a statewide initiative aimed at improving health outcomes and addressing unmet health needs in Ohio's rural communities. Job duties include:

    • Provide program consultation, technical assistance, and ongoing support to RHTP‑funded communities.

    • Review programmatic and budget reports for compliance and prepare summaries for state and federal partners.

    • Monitor funded entities through site visits, phone communication, and reporting forms to ensure eligibility and adherence to program requirements.

    • Develop and present program materials such as fact sheets, applications, guidance documents, and web content.

    • Assist communities in achieving objectives related to diabetes prevention, hypertension management, improved nutrition, and increased physical activity.

    • Plan and facilitate workshops, trainings, conferences, and technical assistance sessions based on identified community needs.

    • Assess rural community needs by evaluating outcome and funding reports, identifying trends, and connecting organizations to resources.

    • Support grant development, contract management, invoice review, budget revisions, RFP creation, and funding recommendations.

    • Maintain program guidelines, policy manuals, required records, newsletters, and reports.

    • Respond to inquiries from healthcare professionals, program participants, government officials, and the public.

    • Utilize GMIS 2.0, WISE, Clear Impact, and other systems to analyze data and generate reports.

    Normal working hours are Monday through Friday 8:00 am to 5:00 pm. This is an hourly position, with a pay range of 12 on the 1199 Pay Range Schedule.

    Position Qualifications:

    Completion of undergraduate core program in social or behavioral science, health or pre-medicine or comparable field; AND 18 mos. exp. in delivery of human services, health care services or medical assistance in governmental, community or private human support services agency or medical provider.

    • Or completion of graduate core program in social or behavioral science or medicine-related or comparable field; AND 6 mos. exp. in delivery of human services, health care services or medical assistance in governmental, community or private human support services agency or medical provider.

    • Or equivalent of Minimum Class Qualifications for Employment noted above.

    • Applications of those who meet the minimum qualifications will be further evaluated against the following criteria:

    • Bachelor's or advanced degree in social or behavioral sciences, medical or health related field

    • Certified Health Education Specialist

    • Experience planning and managing population health programs related to diabetes, hypertension, nutrition and/or physical activity

    • Experience providing consultation/technical assistance to health care and/or community health centers in rural areas to improve patient outcomes and workflows

    • Experience aligning programmatic strategies with intended health-behavior outcomes

    • Experience in program budgeting

    • Experience developing performance measures

    • Experience managing grants and providing technical assistance to grantees

    • Experience analyzing program evaluations and making adjustments to programs

    • Experience writing technical reports, articles, web content and/or correspondence

    • Experience preparing and delivering presentations

    EDUCATIONAL TRANSCRIPT REQUIREMENTS:

    Official educational transcripts are required for all post-high school educational accomplishments, coursework or degrees claimed on the application. Applicants will be required to submit an official transcript prior to receiving a formal offer of employment. Failure to provide transcripts within five (5) working days of being requested will cause the applicant to be eliminated from further consideration. Please note that a transcript is considered "official" only if it is an original copy from the educational institution and includes an institutional watermark, ink stamp or embossed stamp. Transcripts printed from the institution's website will not be accepted. ODH reserves the right to assess the academic credibility of an educational entity's award of a putative degree.
