Rooted in Grace Foundation - Heal & Home Corps

AmeriCorps members at Rooted in Grace Foundation provide compassionate, non-clinical support to pregnant and parenting women and their children as they work toward stable housing, improved health, and long-term self-sufficiency. Members assist with housing stabilization, care planning, healthcare navigation, benefits enrollment, appointment coordination, transportation, and referrals to community resources. They also provide parenting education, life skills coaching, recovery support, child development guidance, and ongoing encouragement through regular, relationship-based interactions. Working alongside healthcare professionals, behavioral health providers, and peer support specialists, AmeriCorps members help families overcome barriers, stay engaged in essential services, strengthen family stability, and build healthier futures through trauma-informed, family-centered support.
