Status: Full-Time, non-exempt

Salary Range: $55,000

Reports To: Director of Behavioral Health

Position Summary

The Case Manager/QMHP provides comprehensive case management and care coordination services to individuals living with an OUD.

Essential Duties and Responsibilities

• Conduct comprehensive assessments and develop individualized service plans (ISPs) based on participant strengths, needs, preferences, and recovery goals.
• Provide ongoing case management and care coordination to assist participants in achieving treatment and recovery objectives.
• Coordinate services among healthcare providers, behavioral health professionals, community agencies, and other support systems to ensure continuity of care.
• Assist individuals and families in accessing benefits and community resources, including Medicaid, housing, transportation, employment, education, and other supportive services.
• Conduct community outreach and educate community members, referral sources, and partner organizations regarding available behavioral health services.
• Distribute approved program supports and incentives, including transportation assistance and food resources, as appropriate.
• Perform other duties as assigned in support of behavioral health and grant-funded services.

Requirements

Minimum Qualifications

• Bachelor's degree or higher from an accredited college or university in Social Work, Psychology, Counseling, Human Services, or a related behavioral health field.
• Current Qualified Mental Health Professional (QMHP) certification required.

Knowledge, Skills, and Abilities

• Knowledge of behavioral health and substance use disorders, trauma-informed care, recovery-oriented practices, and community resources.
• Strong assessment, interviewing, care coordination, documentation, communication, organizational, and problem-solving skills.

Working Conditions

• Monday through Friday, 8:00 a.m. to 5:00 p.m., with occasional evening or weekend activities as needed.
• Travel up to 15% for community outreach, home visits, and coordination of services.
• Work is performed in office, clinic, and community settings.
