Description:
• The Registered Nurse Assessment Coordinator is responsible for conducting federally mandated healthcare assessments in a long-term care setting.
• This full-time role is based in Philadelphia, PA, and requires a professional who can develop and manage individualized nursing care plans.
• The position involves working with various healthcare technologies to ensure comprehensive resident care.
Requirements:
• Strong knowledge of healthcare assessment protocols and procedures.
• Proficiency in managing clinical documentation and coding practices.
• Ability to communicate effectively with interdisciplinary teams and insurance companies.
• Experience in developing and implementing individualized care plans.
• Capability to educate team members on assessment and care plan processes.
