The specialty team coordinator is responsible for providing advanced clinical leadership and clinical outcomes within the ongoing management of patients by facilitating operations, improving system processes, and using evidence-based patient care. This is facilitated by 1) the management of the delivery of quality patient care, 2) The
facilitation of efficient operations to support the delivery of patient care, 3) identify and recommend patient initiatives to promote patient safety and quality, 4) the ongoing management of regulatory and organizational compliance requirements. Responsible for performing all duties and compliance with the Bridgeport hospital mission vision
values and customer service standards.

Responsibilities
Collaborates with clinical nurses assuring patient care across the continuum
Serves as a mentor to clinical staff empowering their advocacy of patients care
Promotes the use of scientific principles and critical thinking skills in the delivery of nursing care
Works with the staff on developing time management prioritization and critical thinking skills
As a leader in and facilitate staff engagement and innovation and evidence-based care
Develop strong positive relationships with all members of the team
Works in collaboration with nursing education to facilitate training of staff
Shares accountability for safe effective high-quality care and optimal outcomes
Uses appropriate teaching learning principles strategies and materials as well as current research and technologies to facilitate the learning of all members of the patient care team
Proctors and mentors skilled communication
Practices and mentors collaboration with interdisciplinary teams
Serves as a resource by providing expert clinical knowledge
Evaluates the current status and needs of a given patient population
Collaboratively establishes an evidence based evolving plan for the improvement of care
Overseas a patient centered environment
Unit specific focus on best practice and barriers
Collaborates with interdisciplinary team to achieve all patient specific goals
Identifies future quality projects that add value through improving hospital cost and patient care
Upholds the standards of specialty practice applicable to the patient population
Participates in continuing education for self in the development of future colleagues
Develops personal goals for professional development and continuing education
Qualifications
EDUCATION: BSN required. Master's Preferred
EXPERIENCE: Three years minimum experience in the area of clinical specialization required.
ACCOUNTABILITY: Demonstrates excellence in RN and Charge roles, Performs any and all RN and Charge RN responsibilities as
necessary to facilitate [patient care and operational efficiency.
LICENSURE/CERTIFICATION: RN License. BLS/ACLS required.
Additional Information
BSN required
3 Years minimum experience in Cardiology. Cardiac Testing required.
Current Connecticut Nursing Licensure.
