Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care
RN Resource/Case Manager.
Coverage needs in the Somerville, Arlington, Belmont, and Watertown, MA areas.
· 32 hours/week
· No weekends required
· Mostly admissions
· Highly competitive pay rates + IRS mileage reimbursement
· 13 week contracts (Possible to extend or convert to perm)
The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
