RN - Residence

We are seeking a Weekend RN Case Manager for Western Cuyahoga (Avon, North Ridgeville, Elyria), Eastern Lorain County (Westlake, North Olmsted, Olmsted Falls), Lake County ((Mentor, Willoughby, Eastlake, Willowick, Kirtland, Painesville), and/or Summit County ((Akron, Fairlawn, Barberton, Cuyahoga Falls, Tallmadge).

RN Case Managers oversee the entire care of the patient beginning to end while under the care of Residence Home Care.

· Provide care where ever the patient calls home.

· Over see the plan of care as directed by the patient's physician.

· Work with the physician for medication needs and changes to the plan of care.

· Manage other disciplines such as LPNs, aides and therapy staff to give the best care needed for each patient.

· Ability to complete OASIS, recert, d/c, ROCs documentations within an EMR System. (DeVero)

· Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.

· When appropriate, teach and counsel family members about care and process for disease/injuries.

· Attend meetings, CEUs in the North Olmsted, Ohio office on a regular basis
