Job Duties



Facilitates entrance of new patients into care programs. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.




This is a grant funded role. If in the future the grant funding is no longer available, this role may be eliminated.




The role is 40 hours weekly; Monday through Friday; 8:00 a.m.- 4:30 p.m.




A minimum of 2 years RN work experience is required.

Job Duties:



+ Responsible for the coordination of pre and post hospital care.


+ Serves as the liaison between the patient and caregivers.


+ Coordinates appropriate multidisciplinary clinics.


+ Performs duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the multi-disciplinary program.


+ Facilitates entrance of new patients into care programs.


+ Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.


+ Provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.


+ Collaborates with other healthcare providers in coordinating and implementing the patient's plan of care including evaluation, treatment, follow-up and surveillance.


+ Communicates with patient via mail, telephone, and in person regarding information pertinent to their care Coordinates patient and interdisciplinary activities to meet the needs of a defined group of patients.


+ Acts as an effective resource.


+ Coordinat's patient activities through the interdisciplinary clinics as appropriate.


+ Reinforces patient and family education based on individual needs either in person or by telephone.


+ Coordinates patient activities through available support services, such as support groups and social services as appropriate.


+ Maintains systems and processes to assure timely follow-up and surveillance of post-follow-up patients.


+ Provides nursing care and assists physician and procedures.


+ Educates other team members in area of expertise.

Education


Graduate from Specialty Training Program- (Required), Bachelor's Degree-Nursing (Preferred)

Experience


Minimum of 2 years-Nursing (Required)

Certification(s) and License(s)


Basic Life Support Certification - Default Issuing Body; Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania
