RN - Quality Pulmonary Coordinator

Summary of Position:
Responsible for coordinating, monitoring, and improving the quality of care for patients with pulmonary conditions across the continuum of care. Serves as a clinical quality expert, ensuring compliance with regulatory standards, evidence-based guidelines, and organizational quality initiatives related to pulmonary care. Partners with physicians, advanced practice providers, respiratory therapy, nursing staff, transitional care coordinators and leadership to drive performance improvement, data integrity, and positive patient outcomes.
Serve as a patient/family advocate ensuring the patient receives optimal care and services throughout the transition from inpatient to outpatient, within the health care system. Will assist in alleviating barriers to care, decrease hospital length of stay, improve adherence to treatment regimen, reduce readmissions, improve patient access to care for the promotion of quality and cost-effective outcomes.
Minimum Qualifications:
a. Bachelor of Science in Nursing (BSN)
b. Current license to practice in the State of Ohio (Registered Nurse license)
c. CCRN (Critical Care Registered Nurse), CPHQ (Certified Professional in Healthcare Quality), or pulmonary-related certification (or willingness to obtain) preferred.
a. Three (3) years of clinical nursing experience.
i. Within pulmonary, critical care, or medical-surgical preferred.
b. Experience in quality, performance improvement, or care coordination roles.
c. Experience with regulatory readiness, accreditation surveys, quality registries and CMS reporting.
a. Knowledge of quality improvement methodologies (e.g., PDSA, Lean, Six Sigma concepts).
b. Strong analytical, organizational, interdisciplinary collaboration and communication skills.
c. Proficiency with electronic health records and data reporting tools.
d. Ability to work well within a team environment by accepting and offering honest and constructive feedback.
e. Organize and manage time to accurately complete tasks within designated time frames in fast paced environment
f. Ability to analyze, use clinical research outcomes, and aggregate data effectively
g. Ability to be open-minded, objective and self-directed in executing responsibilities within a team structure. Proactive approach to problem solving.
a. Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently.
b. Ability to walk long distances within the hospital campus.
c. Ability to sit and/or stand for long periods.
d. Manual dexterity to operate computer, phone, and standard office machines.

The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay.
