About the Role:Prestige Home Health Care LLC is looking for a compassionate and skilled Registered Nurse (RN) to join our dedicated team in Phoenix, AZ. Responsibilities:- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and skilled nursing procedures in a home setting
- Monitor and document patient progress, vital signs, and changes in condition
- Coordinate care with physicians, therapists, and other healthcare team members
- Educate patients and family members on disease management, medications, and home care techniques
- Ensure compliance with state, federal, and agency regulations and standards of care
- Maintain accurate and timely clinical documentation using electronic health records (EHR)
Requirements:- Active Arizona RN license in good standing
- Current BLS/CPR certification required
- Minimum 1 year of clinical nursing experience; home health experience preferred
- Strong knowledge of skilled nursing procedures, wound care, and IV therapy
- Excellent communication, time management, and organizational skills
- Ability to work independently and manage a patient caseload efficiently
- Valid driver's license and reliable transportation for home visits throughout Phoenix, AZ
