Patient Care Manager – Norwood

Schedule: Full-time | Monday – Friday | 8 a.m. to 5 p.m.

Key Responsibilities

• This PCM position is leading the in field clinical team
• Provide clinical supervision to ensure patient care meets professional standards, agencypolicies, and regulatory requirements
• Coordinate and monitor patient care delivery
• Supervise clinical staff to ensure consistent, appropriate service
• Maintain clear communication with patients, caregivers, referral sources, and field and office staff
• Drive quality outcomes by meeting department goals and supporting improvement initiatives
• Model professionalism and organizational values in daily work
• Uphold compliance through required training, accurate reporting, and cooperation with audits
• Promote a safe work environment by following established safety protocols
• Support staff growth through coaching and continuing education

Patient Care Manager Qualifications

• Must have a License in the state or an expired licence from the state that can be reinstated would work.
• Previous experience in home care setting with two years management or supervisory experience, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
