RN - Patient Care Coordinator

The ideal candidate will have PCC or case manager experience, strong Epic charting skills, and experience working with Medicare and Medicaid patients. Seeking an experienced Registered Nurse (RN) / Patient Care Coordinator to provide patient care coordination and case management support.

Job Title: RN - Case manager/Patient Care Coordinator 
Location: Portland, OR -  Hillsboro, OR (Onsite)
Contract Length: 13 Weeks (Possible Extension)
Local Pay: $Best in market (Stipend also available for traveler)
Shift: 08:00 AM – 05:00 PM (8 hrs. x 5 days)
 
Job Summary
Seeking an experienced Registered Nurse (RN) / Patient Care Coordinator to provide patient care coordination and case management support. The ideal candidate will have PCC or case manager experience, strong Epic charting skills, and experience working with Medicare and Medicaid patients.

Job Duties

• Coordinate and manage patient care plans to ensure quality and continuity of care.
• Perform patient assessments and collaborate with physicians, healthcare teams, and other providers.
• Maintain accurate and timely documentation using Epic electronic medical records.
• Coordinate services and resources for patients covered by Medicare and Medicaid.
• Communicate with patients, families, caregivers, and multidisciplinary teams regarding care needs.

Requirements

• Oregon & WA RN or compact license active.
• Must have 1 year of case manager or PCC experience.
