Registered Nurse or Social Worker: Discharge Planning:
Hours: Day shift 8:00am to 4:30am. (No On-Call)
Where: Springfield MA
If you are a Registered Nurse or Social Worker and are looking for "normal business" hours. This could be the role. You will work with a few different departments to help with the discharge planning process. Speak with peers, patients and families during the discharge and follow them through the process.
Required: - A Mass License in good standing:
- A few years of clinical experience.
- Worked with patients and families.
- Case management or U/R experience (this role is just DP)
