POSITION PURPOSE
The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines and standards of NFP and Holy Cross Health. Nurse home visitors report to the VP of Community Health and Well-Being with a dotted line to the NFP Nurse Supervisor at Broward Regional Health Planning Council. Holy Cross is a subrecipient of BRHPC's NFP grant program.

(For patient care providers) Provides nursing care, ensures an environment of patient safety, promotes evidence-based practice and quality initiatives and exhibits professionalism in nursing practice within the model of the ANCC Magnet Recognition Program.
Collaborates with internal and external partners to build diverse and trusting relationships to achieve common goals.
Works collaboratively with all members of the health care team to plan, develop and implement educational, clinical and support services, programs and special events.
Tracks and reports outcomes as required to meet grant objectives.
Responsible for development and implementation of programs and services, oversight of the programs and reporting relationships of the program providers.
Networks with community-based organizations for potential partnerships and volunteers.
Establishes and maintains strong, collaborative working relationships with internal and external stakeholders to ensure resources are available for program development and implementation.
