JOB SUMMARY

Guides and assists the patient through the complexities and barriers of the healthcare system. Assists with all aspects of coordination of care. Implementation and prioritization of patient's problems and needs; and development / implementation / evaluation of a plan of care. Demonstrates critical thinking to assure the delivery of safe, quality, age specific care. Delegates effectively to appropriate team members. Documents accurately and in a timely manner. Communicates and collaborates with other healthcare team members. Adheres to organizational policies and procedures; regulatory accrediting body requirements; and professional practice standards.

Requirements Include

• Graduate of an accredited program for nursing education- BSN required preferred.
• Current licensure to practice professional nursing in the State of Ohio
• Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.
• Clinical skills and expertise in the area of cardiology and heart failure required.
• CHFN or CHFN-K within I year of hire
