    NHC HealthCare Anniston is seeking an organized, compassionate, and detail-oriented RN MDS Coordinator to join our interdisciplinary care team. This is an excellent opportunity for an RN who wants to expand their knowledge of MDS, PDPM, care planning, clinical reimbursement, and long-term-care operations.

    Previous MDS experience is preferred but is not required. Registered Nurses with strong long-term-care experience and an interest in developing their MDS expertise are encouraged to apply.

    Position Overview

    The RN MDS Coordinator coordinates and completes resident assessments according to applicable requirements and established timelines. You will work closely with the interdisciplinary team to promote accurate documentation, individualized care plans, appropriate reimbursement, and high-quality resident care.

    Key Responsibilities

    • Coordinate, complete, and document accurate resident assessments
    • Maintain the MDS assessment calendar and monitor completion deadlines
    • Review clinical documentation for accuracy, completeness, and consistency
    • Interview residents and family members to gather relevant clinical and care information
    • Collaborate with nursing, therapy, dietary, social services, and other departments
    • Complete assessments, Care Area Assessments, and individualized care plans
    • Communicate assessment requirements and deadlines to department leaders
    • Follow up on incomplete or missing documentation
    • Support quality outcomes and appropriate reimbursement through accurate MDS and PDPM processes
    • Participate in interdisciplinary care-plan meetings
    • Assist with on-call responsibilities as needed

    Qualifications

    • Current and active Registered Nurse license required
    • Previous nursing experience preferred
    • Long-term-care or skilled-nursing experience is strongly preferred
    • MDS and PDPM experience is preferred but not required
    • RAC-CT, RAC, or CMAC certification is a plus but not required
    • Strong organizational and time-management skills
    • Excellent written and verbal communication skills
    • Ability to manage deadlines and multiple assessments
    • Strong clinical judgment and attention to detail
    • Ability to collaborate effectively with an interdisciplinary team
    • Compassionate, dependable, and committed to quality resident care
