Lake Ridge Center is an Alzheimer's/Dementia specialty care center. Our center is completely secured, with each unit also secured. Each unit averages 28-36 patients, mostly long-term care. We are looking for a evening shift RN (2p-1030p) working a 4 on, 2 off rotation. There is potential for overtime.

Responsibilities

  • Provide nursing care to residents in LTC settings with accuracy and empathy.

  • Monitor and document vital signs, meds, and treatment responses timely.

  • Ensure compliance with regulatory standards and facility policies.

  • Administer medications and manage med administration records accurately.

  • Collaborate with interdisciplinary team to develop and adjust care plans.

  • Identify and report potential safety risks and quality concerns promptly.

  • Document care activities and maintain precise, up-to-date charting.

  • Utilize care pipelines and metrics to track resident progress and outcomes.

  • Demonstrate accountability for own performance with goal-oriented focus.

  • Engage in ongoing professional development to advance clinical skills.

  • Educate residents and family members on care plans and health maintenance.

  • Maintain on-site presence and collaborate within a high-visibility team environment.


Experience/Qualifications

  • Registered Nurse with current state license and LTC experience

  • Administer medications and manage MARs accurately per facility policy

  • Accurate vital signs monitoring and timely documentation

  • Knowledge of regulatory standards and facility policies

  • Interdisciplinary collaboration to develop and adjust care plans

  • Quality and safety focus with proactive risk reporting

  • Accurate charting and use of care pipelines to track outcomes

  • Accountability for personal performance and goal attainment

  • On-site presence in a high-visibility, target-driven environment

  • Commitment to ongoing professional development and skill advancement
