The Home Health RN is a highly independent, field‑based role responsible for conducting in‑home assessments for PCA (Personal Care Assistant) and MDC (Major Diagnostic Category) services. This position requires strong clinical judgment, excellent documentation skills, and comfort working with Medicaid members across all age groups. This role involves no hands‑on patient care; all work is assessment‑based and completed on a laptop.
Home Health background required
80% field work / 20% work‑from‑home
RN self‑schedules directly with members
Territory: North NJ, primarily Hudson County
Training: First 2 weeks onsite in Hopewell, NJ
Mileage reimbursement: Door‑to‑door at government rate
Schedule: Full‑time, 8‑hour days between 7am–6pm
Also open to RNs available 3–4 full days/week
Phone and laptop provided
Potential for permanent conversion
RN‑only role (LPNs not eligible)
Valid driver’s license and reliable transportation required
Conduct PCA assessments in members’ homes using established clinical guidelines
Complete minimum 5 PCA assessments per day
Evaluate necessity, appropriateness, and efficiency of requested services
Coordinate and assist in implementing services to meet member needs
Collaborate with members, families, physicians, hospitals, and external partners
Support delivery of high‑quality, cost‑effective care aligned with clinical guidelines
Advocate for members across care settings to ensure proper resource utilization
Refer members to MLTSS when appropriate
Document all assessments accurately and comprehensively per policy
Communicate with multidisciplinary teams via phone or in person
Participate in Medical Fair Hearings as needed
Apply fiscal accountability principles in care evaluation
Contribute to quality improvement initiatives
Active NJ RN license or Compact RN license
Minimum 2 years of Home Health nursing experience
Valid driver’s license and reliable transportation
Strong organizational skills and attention to detail
Tech‑savvy; proficient in Microsoft Office Suite, including Excel
Self‑starter with strong work ethic and ability to work independently
