Position Overview
We are seeking a Hospice RN to provide skilled nursing, symptom management, and supportive care to hospice patients and families in home and facility settings.

Required Qualifications

  • Hospice nursing experience.

  • Ability to provide patient-centered care in home and facility settings.

  • Strong assessment, communication, and clinical documentation skills.

  • Ability to work effectively with an interdisciplinary team.

Responsibilities

  • Assess patients and respond appropriately to changes in condition.

  • Provide compassionate nursing care and pain and symptom management.

  • Communicate with patients, families, caregivers, physicians, and other care providers.

  • Complete patient admissions and required documentation accurately and timely.

  • Document skilled care, patient teaching, interventions, and patient/caregiver responses.

  • Obtain and clarify valid physician orders for care provided.

  • Explain hospice philosophy, services, and the role of hospice care to patients, families, and facility staff.

  • Implement the Plan of Care under physician direction and applicable regulations.

  • Participate in interdisciplinary patient case conferences as needed.

  • Supervise Hospice Aides every 14 days as required.

  • Assist with orientation and mentoring of new staff when requested.

  • Manage time efficiently and prioritize patient needs.

  • Perform additional responsibilities as assigned.
