Position Overview

Seeking an experienced and compassionate Hospice Registered Nurse (RN) Case Manager to provide comprehensive end-of-life care to patients in their homes throughout the Seven Hills, Ohio service area. The RN Case Manager is responsible for coordinating and managing all aspects of patient care, conducting comprehensive assessments, developing individualized care plans, collaborating with the interdisciplinary team, and providing clinical support and education to patients and families. This role requires strong hospice case management experience, proficiency with Homecare Homebase (HCHB), and the ability to manage an independent field-based caseload.

Schedule & Work Details

  • Location:  Seven Hills, OH 44131
  • Schedule: Full-time, 40 hours per week
  • Work Days: Monday through Friday
  • Shift: 8:00 AM - 4:30 PM
  • Availability Requirement: Must be available during scheduled hours
  • On-Call Requirement: None
  • Travel Requirement: Yes, 50% to 75% travel between patient visits

  • Pay Rate: $51.00 - $53.00 per hour

Key Responsibilities

  • Manage a caseload of hospice patients and serve as the primary clinical point of contact throughout the patient's hospice journey.
  • Conduct comprehensive nursing assessments and ongoing evaluations of patient condition, symptoms, and care needs.
  • Develop, implement, and update individualized hospice plans of care in collaboration with the interdisciplinary team and attending physician.
  • Provide skilled nursing care and symptom management for patients with terminal illnesses.
  • Assess and manage pain, comfort measures, and end-of-life symptoms to promote quality of life.
  • Educate patients, families, and caregivers regarding disease progression, symptom management, medications, hospice services, and available resources.
  • Coordinate care with physicians, social workers, hospice aides, chaplains, therapists, and other members of the care team.
  • Monitor patient status and communicate significant changes in condition to the appropriate providers.
  • Complete accurate and timely documentation in Homecare Homebase (HCHB).
  • Participate in interdisciplinary team meetings, case conferences, and care planning discussions.
  • Provide emotional support and guidance to patients and families throughout the hospice experience.
  • Ensure compliance with hospice regulations, Medicare guidelines, agency policies, and professional nursing standards.
  • Coordinate admissions, recertifications, and discharges as needed.
  • Supervise and collaborate with hospice aides and other support staff involved in patient care.

Required Qualifications

  • Active Registered Nurse (RN) license.
  • Current CPR certification.
  • Previous Hospice RN Case Management experience.
  • Experience using Homecare Homebase (HCHB).
  • Valid driver's license, reliable transportation, and proof of insurance.
  • Ability to travel extensively throughout the service area.
  • Strong clinical assessment, critical thinking, and care coordination skills.
  • Excellent communication and documentation abilities.

Preferred Qualifications

  • Strong symptom management and end-of-life care expertise.
  • Experience managing an independent hospice caseload.
  • Exceptional patient and family education skills.
  • Ability to build trust and rapport with patients, caregivers, and interdisciplinary team members.
  • Knowledge of Medicare hospice regulations and compliance standards.
  • Compassionate, empathetic, and patient-centered approach to care.
