Position Overview
Seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our hospice care team. This role is responsible for coordinating and managing patient care, completing admissions, collaborating with the interdisciplinary team, and ensuring patients receive high-quality end-of-life care in accordance with hospice standards and regulations. The ideal candidate will be flexible, team-oriented, and passionate about providing exceptional patient-centered care.
Schedule & Work Details
- Location: Coverage area includes Shelby, Fayette, and Tipton counties
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- On-Call/Weekend Requirement: None
- Productivity Expectation: 6 visits per day / 30 points per week
- Mileage Reimbursement: Yes, mileage is reimbursed between patient visits from first patient to last patient
- Pay Rate:$51.00 - $53.00 per hour
Key Responsibilities
- Manage and coordinate hospice patient care plans throughout the continuum of care
- Complete hospice admissions and ensure timely initiation of services
- Conduct comprehensive patient assessments and ongoing evaluations
- Develop, implement, and update individualized plans of care
- Prepare for and participate in Interdisciplinary Team (IDT) meetings
- Collaborate with physicians, social workers, chaplains, hospice aides, and other healthcare professionals
- Provide patient and family education regarding disease progression, symptom management, and hospice services
- Maintain accurate and timely clinical documentation within the electronic medical record
- Ensure compliance with hospice regulations, agency policies, and quality standards
- Advocate for patients and families while delivering compassionate end-of-life care
Required Qualifications
- Current active Registered Nurse (RN) license
- Previous hospice nursing experience required
- Proficiency with Homecare Homebase (HCHB) required
- Strong assessment, care coordination, and case management skills
- Excellent communication and interpersonal abilities
- Valid driver's license and reliable transportation for travel throughout the assigned territory
